Trigeminal sensory neuropathy This is a rare condition, in which there is pain and sensory loss either unilaterally or bilaterally on the face, and for which no cause can be found, despite intensive investigation. VII cranial nerve (facial nerve) This is a predominantly motor nerve, although there is a small sensory component supplying sensation to the external acoustic meatus. Running in conjunction with the nerve is the chorda tympani, which carries sensation from the anterior two thirds of the tongue.

When the peripheral sensory nerve is disturbed, initially the patient will describe tingling or pins and needles (paraesthesiae) which then progresses to loss of sensation (numbness). With involvement of the dorsal root entry zone, the patient describes the whole limb as feeling numb and ‘tingly’ and is often particularly aware of loss of temperature sensation, so that putting the leg into a bath does not give the same appreciation of temperature as immersing the rest of the body. Within the spinothalamic pathways, in addition to the symptoms of the dorsal root entry zone, the patient often describes that the pins and needles are ‘burning’ in character.
