It would have taken Mary Ann some time in classical psychoanalysis to realize that her symptoms of distress did have meaning and were a signal of anxiety. It would have taken even longer to "cure" her or make her feel more comfortable. Today, her case can be diagnosed and treated in a much shorter period of time. Anxiety is an enigma. Its exact definition remains elusive due to the fact that it is used in a variety of ways depending on the author's perspective.

What Is Panic Disorder? Patients often complain of anxiety and tension. But panic disorder is different. As you can see from the above clinical vignette, anxiety reaches the level of a panic attack when it is overwhelming and associated with personality disorganization, an inability to function, and signs of intense arousal of the autonomic nervous system. A full-blown panic disorder is characterized by panic symptoms or attacks that occur with enough frequency to be disabling or impairing. "1 Definition: DSM-IH-R Criteria According to the Diagnostic and Statistical Manual of Mental Disorders (DSM-III-R),2 published in May 1987, the diagnostic criteria for panic disorder indicate that when these attacks of sudden and intense anxiety occur four times in a four-week period (or one or more attacks have been followed by a period of at least a month of persistent fear of having another attack), the attacks have progressed to a panic disorder.
